<objective> Verify threat mitigations for a completed phase. Three states:
- (A) SECURITY.md exists — audit and verify mitigations
- (B) No SECURITY.md, PLAN.md with threat model exists — run from artifacts
- (C) Phase not executed — exit with guidance
